How to Enroll

A parent may contact JBA directly to inquire about our services. In addition, a school district, regional center, or insurance representative, on behalf of the parents, may also contact JBA to initiate services.

Initial intake information will be gathered and an information packet will be mailed or emailed to the family.

Parents/caregivers will need to fill out a Client Questionnaire and return it to JBA.

Once we have received this form, the family will be placed on the waitlist and notified regarding the availability of services.
