How to Enroll

• A parent may contact JBA directly to inquire about our services. In addition, a school district, regional center, or insurance representative, on behalf of the parents, may also contact JBA to initiate services.

• Initial intake information will be gathered and an information packet will be mailed or emailed to the family.

​

• Parents/caregivers will need to fill out a Client Questionnaire and return it to JBA.

​

• Once we have received this form, the family will be placed on the waitlist and notified regarding the availability of services.
